Determining outcomes is a vital element of any social, behavioral, or public health initiative. Whether it's for a strategic plan, logic model, grant, or other planning purpose, most change initiatives involve writing outcomes statements. Outcomes should be SMART, that is Specific, Measurable, Achievable, Relevant, and Time-bound.
